
The immunologic mechanisms of sublingual immunotherapy are less established. In Cochrane analysis, the authors concluded that there was an increase in IgG4 but no stable effect on IgE levels in adults. In addition, the induction of allergen-specific IgA has been reported. There are conflicting data concerning lympho-proliferative responses. So far the evidence on changes in Th1/Th2/Treg activity induced by sublingual immunotherapy need to be confirmed. The effects on T-cell reactivity and cytokine secretion show strong variation in a number of studies. (more…)

Atopic Dermatitis is considered to be one of the first manifestations in the atopic march. The aim of this study was to investigate prospectively the natural course of Atopic Dermatitis to determine factors influencing its prognosis and to analyse the relationship of Atopic Dermatitis with childhood asthma. The Multicenter Allergy Study, a German birth cohort, followed 1314 children from birth to age 7 years. Physical examinations, parental interviews on atopic symptoms and diagnoses, and determination of specific immunoglobulin (Ig)E levels were performed regularly. (more…)

The investigators set out to determine whether the middle ear compartment may be a component of the united airways in allergic disease by comparing the inflammatory profiles of the middle ear with those of the upper airway. Middle ear effusions, torus tubaris (Eustachian tube mucosa at the nasopharyngeal orifice) and adenoidal tissue biopsies were obtained from 45 patients undergoing simultaneous tympanostomy tube placement for Otitis media with effusion and adenoidectomy for adenoid hypertrophy. The diagnostic approach to allergic food reactions comprises three steps. The first step includes the medical allergy history, physical examination, and family allergy background. On the basis of symptoms and timing of the reaction, the physician attempts to identify the suspected food and to determine whether the reaction is likely to involve an immunologic mechanism. This first step is absolutely necessary to decide on the subsequent diagnostic tests to be performed. The second step includes allergy skin tests and in vitro assays, which can confirm a sensitization to the food. (more…)

Patient’s allergy history is one of the most important elements to evaluate of possible allergic problem. Sometimes this important factor is overlooked. Background information regarding patient’s allergy history will be used in following physical examination and allergy testing. Patient allergy history is made up of major complaints, determination of seasonality or variations of symptoms, trigger’s identifications, response allergic to medication, exposure in job environment, family allergy history, and other relational medical history. At first, it may not obvious to patient, which past allergy history are important. It is the duty of physician to conduct some questionnaire and highlight what is important history is essential.
