The immunologic mechanisms of sublingual immunotherapy are less established. In Cochrane analysis, the authors concluded that there was an increase in IgG4 but no stable effect on IgE levels in adults. In addition, the induction of allergen-specific IgA has been reported. There are conflicting data concerning lympho-proliferative responses. So far the evidence on changes in Th1/Th2/Treg activity induced by sublingual immunotherapy need to be confirmed. The effects on T-cell reactivity and cytokine secretion show strong variation in a number of studies. (more…)

Once it is tethered onto the venule wall, the neutrophil or other leukocyte comes into contact with a wide variety of inflammatory mediators that may either be expressed by the activated endothelium or simply diffuse into the blood from the injured tissue. Among these mediators are a diverse subset of intermediaries known as leukocyte chemotactic factors which bind to receptors on the leukocyte surface and trigger the second, activation phase of margination. Hematopoietic Differentiation and Cell Growth

Our understanding of hematopoiesis has advanced greatly in recent years with the isolation and characterization of hematopoietic stem cells (HSCs) and the identification of many of the factors that influence the production and differentiation of lineage-committed progenitors (Figure 1 bellow). HSCs are defined by their abilities to self-renew throughout life and to give rise to committed progenitors that can differentiate along all of the possible hematopoietic lineages. They were first purified from mice as a tiny sub-population of marrow cells that could completely reconstitute the hematopoietic systems of other mice, whose own marrows had been destroyed by inherited mutations or by radiation. Allergic Reactions To Penicillin and Skin Test Evaluation via Intradermal Injection

This study compared the diagnostic value of intradermal tests and patch tests in 20 patients with non-immediate reactions to penicillin (none had IgE antibodies to benzylpenicillin or amoxicillin detectable using a commercial RAST [radioallergosorbent test] method), using 30 patients tolerant to penicillin as controls. Intradermal tests assessed reactivity to injection of major and minor determinants of benzylpenicillin, amoxicillin and ampicillin. Allergy patch testing involved the same hapten solutions used for intradermal testing, either embedded in a patch disk or mixed with petrolatum. (more…)

Extrinsic allergic alveolitis (synonym: hypersensitivity pneumonitis) describes an interstitial and bronchioloalveolar disease of the lungs caused by an immune mediated hypersensitivity response. It is associated with repeated exposure to a variety of inhaled organic dusts and reactive chemicals (allergy chemical), but only a proportion of exposed subjects become affected. Notable examples are farmers’ lung (due to actinomycetes contaminating farm produce) and bird fanciers’ lung (due to secreted avian antigens). The disease has considerable potential as a model of allergic lung disease, since the populations at risk can often be identified precisely, the relevant antigens can be purified, and the clinical and immunologic consequences of antigen exposure can be examined.
